Annual report on adopted child from Ukraine

This is the form used for reporting back to the Ukrainian government following the adoption of a Ukrainian child. I created a .docx version of this to make it easy to type in responses and to save basic information for future years. Also the link to the Ukraine consular website was down, so I am including the original PDF version as well.

The reporting schedule last I heard was once a year for the first three years, then every three years until the child is 18 years old.
